Use these forms to keep the HOA and your community informed, improve community safety, and to contact you in case of emergency.
Rentals and Guests Forms
Units may be rented for a minimum of one month and a maximum of six months within a 12-month period. Forms also are needed to have friends and family use your unit. The Owner Contact Form ensures we have your and your guests' latest contact information for emergencies and communications. Completed forms may be dropped off at HOA mailbox in front of Montana Vista pool entrance, mailed to the HOA, 1700 Montana Ave., Lake Havasu City, AZ 86403, Attn: Maria Hart or emailed to bod@loslagosvistashoa.com.
Owners are responsible and accountable to complete these forms.
